All you do is 'add' all the pictures/graphics BEFORE you hit 'post reply'.  However, it's  best NOT to put too many graphics in one comment.  Example:

To add more than one graphic to a comment, copy the IMG Code at Photobucket or your image server and then paste it into the comment section, then find another graphic and copy the IMG Code and paste it directly below the first graphic.  Hit the 'Enter' key before pasting the second graphic.

If you have Smileycons you can simply drag and drop the images into the comments/message/forum post.
